Who might be able to join this trial:

  • Adults aged 18 or older
  • People admitted to hospital with a diagnosis of acute heart failure where the body is retaining excess fluid
  • People showing signs that their water tablets (diuretics) may not be working well enough, which means either having taken daily diuretics by mouth for at least 7 days before admission, or not losing enough fluid in the past 24 hours (less than 500g in weight or less than 1 litre of fluid) despite receiving high doses of diuretics through a drip
  • People showing physical signs of excess fluid buildup, such as swelling in the legs or ankles, fluid in the abdomen, raised pressure in the neck veins, or scan results showing fluid on the lungs

Who may not be able to join:

  • People who are having an acute heart attack or acute coronary syndrome
  • People whose systolic blood pressure (the top number) is below 85 mmHg
  • People who have already received kidney dialysis or a fluid-removal procedure (ultrafiltration) during the current hospital stay before the study begins
  • People who have already been treated with acetazolamide or metolazone (specific types of diuretic medications) during the current hospital stay before being enrolled in the trial
  • People with a known allergy to any of the medications used in the trial
  • People with severely low potassium or severely low sodium levels in their blood
  • People with severe liver disease or liver cirrhosis
  • People who are known to be pregnant
