Who might be able to join this trial:

• Adults between 18 and 80 years of age.
• People who are willing to participate, have signed a consent form, and are able to attend follow-up appointments.
• People whose primary colorectal cancer (bowel cancer) can be fully removed by surgery, and where the diagnosis has been confirmed by a tissue sample analysis.
• People who have been diagnosed with cancer that has spread to both the liver and at least one other area outside the liver, as confirmed by CT or MRI scan, where the spread outside the liver is considered stable and manageable by a specialist team.
• People who have received only first- or second-line (first or second round) systemic treatment for their cancer.
• People whose liver metastases (cancer spread to the liver) can be treated with either: full surgical removal while keeping enough healthy liver tissue, or ablation therapy (a procedure to destroy tumour tissue) where the tumour is smaller than 3 cm and no more than 5 tumours need to be treated — as determined by a specialist team review.
• People with a general health and activity level rated as 0 or 1 on a standard scale (meaning fully active or able to carry out light work).
• People with a life expectancy of at least 3 months.
• People with a liver function score of 7 or below on a standard medical scoring system (Child-Pugh scale).
• People whose routine blood and organ function tests meet all of the following minimum levels (confirm with trial site):
  • Adequate white blood cell (neutrophil) levels in the blood.
  • Adequate platelet levels in the blood.
  • Adequate haemoglobin (red blood cell) levels.
  • Bilirubin (a liver-related substance) within an acceptable range.
  • Liver enzyme levels within an acceptable range.
  • Kidney function within an acceptable range.
  • Adequate protein (albumin) levels in the blood.

Who may not be able to join:

• People whose cancer has spread only to the liver, with no spread to other areas outside the liver.
• People with a history of hepatic encephalopathy (a condition where the liver cannot filter toxins, affecting the brain) or who have had a liver transplant.
• People whose cancer has spread to the brain.
• People who are pregnant or breastfeeding.
• People with a history of another cancer, unless that cancer has been in full remission for at least 5 years — thyroid cancer and carcinoma in situ (very early-stage, non-invasive cancer) may be exceptions (confirm with trial site).
• People who have had a serious heart or stroke-related event (such as a heart attack or stroke) within the past month, or whose heart or cardiovascular condition is not well controlled.
• People with significantly reduced heart function, based on specific heart measurements (confirm with trial site).
• People with high blood pressure that cannot be controlled with treatment, or who have a history of hypertensive crisis or related brain complications.
• People with a serious active infection that cannot be controlled.
• People with significant breathing difficulties or low blood oxygen levels at rest.
• People with a large amount of fluid build-up in the abdomen, or fluid that contains cancer cells or elevated cancer markers.
• People with blood clotting problems or a tendency to bleed.
• People with wounds or fractures that have not healed, or who have had major surgery, a serious injury, a fracture, or an open wound within the past 4 weeks.
• People with a history of substance dependence or mental health disorders that cannot be managed.
• People with other serious conditions that, in the treating doctor's judgement, could put them at risk or prevent them from completing the study.
• People who the investigating doctor considers unsuitable for the trial for other reasons.
