Who might be able to join this trial:
- People who have been diagnosed with primary liver cancer (hepatocellular carcinoma), confirmed by a biopsy or by standard clinical diagnostic guidelines
- People who have a blood clot or tumour invasion in the main vein supplying the liver (called portal vein tumour thrombus), rated as Grade III or IV using a specific medical grading system called Cheng's criteria
- People whose portal vein blockage has caused increased pressure in the surrounding blood vessels (portal hypertension)
- People who may or may not have experienced bleeding from swollen veins caused by this portal vein blockage
- People whose cancer has spread to no more than five spots in total, across no more than two organs outside the liver

- People who are receiving Lenvatinib and a PD-1 inhibitor as their first course of systemic (whole-body) treatment
- People whose liver function is rated as Child-Pugh class A or B, and whose general physical ability is rated 0 or 2 on a standard medical scale (ECOG scale)
- People with no history of any other type of cancer
- People who have agreed to take part in this clinical trial
- People whose blood test results meet specific thresholds for white blood cells, red blood cells, platelets, liver enzymes, and kidney function (confirm with trial site for exact values)
Who may not be able to join:
- People whose liver cancer has come back after previous treatment (recurrent HCC)
- People whose portal vein tumour thrombus is rated as Grade I or II by Cheng's criteria
- People younger than 18 years old or older than 75 years old
- People whose cancer has spread to more than five sites outside the liver
- People with more than five tumours within the liver
- People whose cancer has not responded to Lenvatinib
- People whose life expectancy is estimated to be less than 3 months
