Who might be able to join this trial:
- You are over 18 years old
- You live within approximately 20 km of Gap, Laragne-Montéglin, Veynes, or Clermont-Ferrand in France
- You have been personally invited to take part by staff at a medical facility involved in the trial
- You have been diagnosed with one of the following long-term conditions: stroke, peripheral artery disease (blocked arteries in the legs), heart rhythm problems, heart valve disease, a heart condition present from birth, mild to moderate heart failure (early stages), severe high blood pressure, or chronic coronary disease (ongoing reduced blood flow to the heart)
Who may not be able to join:
- You have been diagnosed with severe heart failure (stages III or IV, meaning significant symptoms during minimal activity or at rest)
- You have a medical reason why physical activity or exercise is not safe for you
- You currently live in a care home or similar institution
- You are unable to walk more than 100 metres on your own
- You have Alzheimer's disease or another diagnosed form of dementia
- You have a cognitive (thinking or understanding) difficulty that makes it hard to understand instructions or information, even if not formally diagnosed (confirm with trial site)
