Who might be able to join this trial:

• People aged 18 to 80 years, of any gender
• People who have been diagnosed with hepatocellular carcinoma (a type of liver cancer), either through clinical assessment or tissue testing
• People whose liver cancer has been classified at a specific advanced stage (CNLC IIb–IIIa) and has been assessed by a specialist team as not suitable for surgical removal
• People who have at least one liver tumor that can be measured on scans, has not yet been treated, and where the largest tumor measures more than 5 cm across
• People who have either not had any systemic drug treatments (such as chemotherapy, targeted therapy, or immunotherapy) for this liver cancer, or who have had such treatments but whose cancer was assessed as stable or progressing during that treatment
• People with a good general health and activity level (ECOG score of 0 or 1) and an expected survival of more than 3 months
• People whose liver function scores within an acceptable range (Child-Pugh score of 7 or below)

Who may not be able to join:

• People with severely reduced liver function (Child-Pugh class C)
• People who have a blood clot or tumor growth blocking the main portal vein or hepatic vein (major blood vessels of the liver)
• People whose cancer has spread widely through the body and whose expected survival is less than 3 months
• People with serious loss of function in major organs, including the liver, kidneys, heart, lungs, or brain
• People who have had bleeding from swollen veins in the food pipe or stomach in the past month
• People who have had a different type of cancer at any point in their life
• People who received any anti-cancer treatment — such as radiation, chemotherapy, or a local procedure — within the past month
• People with an active infection; a hepatitis B infection where the virus level remains high unless significantly reduced by antiviral treatment; a hepatitis C infection that requires antiviral treatment according to medical guidelines; an HIV infection; or inflammation of the bile ducts
• People who have previously had an organ transplant or who have a history of hepatic encephalopathy (a condition where the liver cannot properly filter toxins, affecting brain function)
• People with blood clotting problems that cannot be corrected
• People with severe, treatment-resistant fluid build-up in the abdomen or chest, or significant unintended weight loss and muscle wasting
• People who are pregnant, have impaired consciousness, or would be unable to follow the treatment plan
• People with a high overall tumor burden, where the size of the largest liver lesion (in centimetres) added to the total number of liver lesions comes to more than 12
• People who have any other condition that the trial investigators consider would make participation unsuitable (confirm with trial site)
