- People diagnosed in the past 6 months with one of these cancers at Stage I, II, or III: breast cancer, prostate cancer, urinary bladder cancer, or endometrial cancer
- People whose cancer treatment has the goal of curing the cancer (rather than just managing it)
- People who had their first visit at the cancer centre within the last 3 months
- People receiving curative-intent cancer treatment at the Rutgers Cancer Institute of New Jersey
- People who had at least one of the following health conditions before their cancer diagnosis that requires medication to manage: high blood pressure, high cholesterol, or type 2 diabetes
- People who are currently receiving care from a primary care doctor or provider
- People who speak English or Spanish
Who may not be able to join
Each point below is a reason the trial team may not be able to accept someone. It is not a list of requirements to meet.
- People who have had a heart attack or stroke within the past year
- People who have heart failure where the heart is pumping at less than 30% of its normal capacity
- People who have moderate-to-severe chronic kidney disease (specifically stage IV or V, meaning the kidneys are working at less than 30% of normal function)
- People who do not speak English or Spanish
Note: People whose cancer spreads to other parts of the body during the 18-month trial period may still be allowed to continue participating, unless they choose to withdraw.
