Who might be able to join this trial:

• Adults aged 18 or older.
• People who have been diagnosed with colorectal cancer (a type of bowel cancer) confirmed by a tissue sample (biopsy).
• People whose cancer has spread to the liver only, as confirmed by scans. Small spots outside the liver (1 cm or smaller) that have not been confirmed as cancer spread may still be acceptable.
• People who have not received any previous treatment for their metastatic (spread) cancer, or who finished chemotherapy given before or after surgery at least 6 months before their cancer was found to have come back.
• People whose liver tumours have been assessed by a medical team and considered unable to be surgically removed, or only borderline possible to remove.
• People whose liver tumours can be measured on scans using standard criteria.
• People who are generally well enough to carry out everyday activities with no more than minor limitations (confirmed with trial site).
• People with a life expectancy of at least 12 weeks.
• People who have no medical reasons preventing liver surgery or tumour destruction, and whose original bowel tumour has already been removed or is suitable for removal.
• People whose blood, kidney, and liver test results fall within acceptable ranges as determined by the trial team.

Who may not be able to join:

• People whose cancer has clearly spread to areas outside the liver.
• People whose colorectal cancer has a specific genetic feature known as dMMR or MSI-H (confirm with trial site).
• People who previously could not tolerate the study drug when it was used as part of earlier chemotherapy treatment.
• People who still have side effects from previous cancer treatment that have not improved to a mild level, with exceptions for hair loss (any level) and a specific type of nerve-related tingling or numbness (if mild to moderate).
• People who have had another cancer diagnosis within the past 5 years, unless it was a localised cancer that has been fully cured.
• People who had surgery within 28 days before joining the trial.
• People with high blood pressure that remains at or above 160/100 mmHg even while taking blood pressure medication.
• People who had a serious heart event (such as a heart attack or stroke) within the 12 months before joining the trial.
• People with a history of serious bleeding problems or blood clotting disorders, or whose tumour is located near major blood vessels or shows significant breakdown, where the trial doctors feel there is a meaningful bleeding risk.
• People with an active autoimmune disease (where the immune system attacks the body) that has required medical treatment within the past 2 years.
• People with a weakened immune system, a positive HIV test, or those who have been taking steroid or immune-suppressing medications long-term.
• People who have previously had a bone marrow or organ transplant from another person.
• People who received a live vaccine within 30 days before starting the trial, or who plan to receive one during the trial.
• People who are allergic to any component of the study drugs, or who have had a serious allergic reaction to similar medicines (monoclonal antibodies) in the past.
• People who are pregnant or breastfeeding.
• People with serious other health conditions that could make the treatment unsafe, including significant heart failure, unstable chest pain, uncontrolled heart rhythm problems, severe liver disease, uncontrolled kidney disease, severe stomach ulcers, uncontrolled metabolic conditions, or mental health or social circumstances that could affect safe participation.
• People who the trial doctors consider unsuitable for any other reason.
