- You are willing to read and sign a consent form agreeing to take part

- You have been diagnosed with at least 2 of the following conditions:

- A persistent or permanent irregular heart rhythm called atrial fibrillation (not the occasional/short-lived type)
- A previous stroke caused by a blood clot or blockage
- Blocked arteries in the legs or other limbs (peripheral arterial disease)
- Reduced kidney function meeting specific medical measurements (confirm with trial site)
- Use of a certain type of water/fluid pill (loop diuretic) for more than 30 days within the past year
- Chronic obstructive pulmonary disease (COPD), confirmed by breathing tests, a specialist, scan, or specific treatment
Who may not be able to join:
- You are unable to give informed consent — for example, due to memory or thinking difficulties, limited English, or difficulty understanding the study information
- You have already been diagnosed with heart failure (for any reason)
- You are currently receiving kidney dialysis or another form of kidney replacement treatment
- You are unable to travel to Cleveland Clinic for tests, such as being in hospice or a nursing facility
- You have had a left breast removal and breast reconstruction surgery, as this may prevent use of the AI heart scan tool
- You have only visited Cleveland Clinic once before, suggesting it is not your regular place of care
- In the opinion of the research team, there is another reason you may not be suitable to participate (confirm with trial site)
