Who might be able to join this trial:
- People aged 18 or older.
- People who have at least one artery with a significant plaque buildup, identified by a special heart scan called a CCTA, where the artery is either narrowed by 50% or more, or where plaque takes up 70% or more of the artery wall.
- People whose plaque shows at least 2 of these specific features on the scan: low-density (soft) plaque, artery wall that has expanded outward around the plaque, a "napkin-ring" pattern in the plaque, or small scattered calcium deposits.
- People whose affected artery segment is in one of three main heart arteries: the left anterior descending artery, the left circumflex artery, or the right coronary artery (in the upper or middle section of these arteries).
- People who are able to confirm they understand the risks, benefits, and alternatives involved, and who can provide written consent before the study begins (or have an authorised representative do so on their behalf).
Who may not be able to join:
- People whose target artery has already been treated to restore blood flow, or where such a procedure is already planned.
- People currently experiencing an acute (sudden or severe) heart event, such as a heart attack or unstable chest pain.
- People with severely reduced heart pumping function (specifically, a pumping capacity below 30%) or who have advanced heart failure symptoms classified as Class III or IV by standard medical guidelines.
- People with uncontrolled or repeatedly occurring dangerous heart rhythm problems in the lower chambers of the heart.
- People with a specific inherited cholesterol condition called homozygous familial hypercholesterolaemia.
- People with active liver disease or significant liver problems.
- People whose CCTA scan could not be successfully analysed for plaque.
- People with a non-heart-related illness where life expectancy is less than 2 years.
- People who are pregnant or breastfeeding.
- People with a known allergy or medical reason they cannot take statins or a class of cholesterol-lowering medications called PCSK9 inhibitors.
