Who might be able to join this trial:
- Adults who have been admitted to hospital with worsening heart failure and who may be ready to go home early with the support of a care programme and regular monitoring.
- People with suddenly worsening heart failure who are at home or being seen in an outpatient clinic and need large amounts of fluid-removing medication (diuretics), but whose condition does not require full hospital-level care.
- People with advanced, severe heart failure (known as Stage D) who need their fluid-removing medication adjusted frequently — more than once a week — whether they are currently in hospital or being seen as an outpatient.
Who may not be able to join:
- People whose condition requires the level of monitoring and care that only a hospital can provide.
- People who do not pass a safety assessment, are currently struggling with substance use, or have a mental health diagnosis that could affect their ability to take part.
- People who are currently enrolled in hospice care.
- People who are living in a skilled nursing facility.
- People who do not have a primary care doctor or a heart specialist (cardiologist) at Mayo Clinic.
- People who have difficulty communicating due to a medical condition or problems with memory and thinking.
- People who are pregnant.
