Who might be able to join this trial:
- Adults aged 18 years or older
- People who have been diagnosed with chronic kidney disease
- People who have coronary artery disease, either a type called non-ST elevation acute coronary syndrome (a certain kind of heart attack or chest pain episode) or ongoing symptoms of stable heart disease (confirm with trial site)
- People who can communicate in English or French, or who have a support person with them who speaks English or French
- People who are able to take part in discussions about their care and treatment decisions, either on their own or with the help of a trusted person who can make decisions on their behalf, as determined by their treating doctor
Who may not be able to join:
- People whose kidneys have failed to the point where they are already receiving dialysis, or whose kidney function test result is below a very low threshold (eGFR less than 10 mL/min/1.73m²)
- People who have had a specific type of serious heart attack called an ST-elevation myocardial infarction
- People who are not expected to survive until they leave the hospital
