Who may be able to join
Care Partners (family members, friends, or partners supporting a Veteran):
- A relative, friend, or partner aged 18 or older who has a personal relationship with a Veteran and regularly helps them with care or organising their care
- Living in a rural area, based on official rural classification codes (Rural-Urban Continuum Codes)
- Must be enrolled in the VA's General Caregiver Support Services program
- Can live with or separately from the Veteran, and must be able to communicate in English by phone
Veterans:
- Receiving care at the Durham, Asheville, or Richmond VA Health Systems, with at least 2 outpatient visits in the past year and an assigned primary care provider, and living in a rural area
- Diagnosed with congestive heart failure, chronic obstructive pulmonary disease, cancer, dementia, or end-stage renal disease
- Needs help with at least one basic daily activity (such as walking, eating, using the toilet, bathing, getting dressed, or moving from place to place) or one practical daily task (such as transport, managing medications, handling finances, shopping, or preparing meals)
- Aged 50 or older and able to communicate in English by phone for study assessments
