- Your heart's pumping ability is significantly reduced, meaning tests show your heart pumps out 35% or less of blood with each beat
- You have had an implanted defibrillator device (ICD — a small device placed in your chest to help control dangerous heart rhythms) for at least 12 months

- Your heart or coronary artery disease is currently unstable or not well controlled
