Who might be able to join this trial:

• Adults between 18 and 75 years of age
• People who have been diagnosed with a liver or bile duct cancer (such as hepatocellular carcinoma or cholangiocarcinoma) and whose doctors have assessed them as suitable for a specific two-stage liver surgery called ALPPS
• People whose liver is functioning well enough before surgery, as measured by specific medical scores (Child-Pugh A or B, and a MELD score of 15 or below)
• People whose cancer affects both sides of the liver or is otherwise complex, meaning the tumour is large enough that a special surgical approach is needed to leave enough healthy liver tissue behind
• People whose scans show that the portion of liver expected to remain after surgery is too small for a standard operation, making the two-stage ALPPS approach necessary
• People whose remaining liver has grown sufficiently (at least 30% larger, or meeting a specific size-to-body-weight ratio) after the first stage of surgery, making the second stage possible
• People who do not have cancer that has spread to other parts of the body in a way that cannot be surgically removed — although having lung spread that can be surgically removed may still be acceptable (confirm with trial site)

Who may not be able to join:

• People whose remaining liver did not grow enough after the first stage of surgery to safely proceed to the second stage
• People whose cancer has spread to distant parts of the body, such as extensive spread to the bones or lungs, in a way that cannot be surgically removed
• People with very poor liver function before surgery, rated as Child-Pugh grade C, or those found during surgery to have severe liver scarring or significant fat build-up in the liver
• People with serious ongoing health conditions — such as severe heart disease, kidney failure, or respiratory failure — that make major surgery too risky
• People who experienced serious complications after the first stage of surgery, such as liver failure or serious infection, that make the second stage unsafe
• People whose remaining liver shows problems with blood supply or function after the first stage that are affecting the liver's ability to regrow (confirm with trial site)
• Women who are pregnant or breastfeeding
