Who might be able to join this trial:
- People aged 18 years or older
- People who have been diagnosed with Acute Decompensated Heart Failure (a sudden worsening of heart failure), regardless of which type they have
- People who show at least one sign of fluid build-up in the body, such as fluid around the lungs (seen on an X-ray or ultrasound), swelling in the legs or fluid in the abdomen (seen on an ultrasound)
- People whose blood test shows a specific heart stress marker (NT-proBNP or BNP) above a certain level at the time of screening
- People who have been taking at least 40 mg of a water tablet called furosemide by mouth every day for at least one month before the study begins
Who may not be able to join:
- People who have previously used the medications acetazolamide or metolazone before the study period
- People who are known to be allergic or sensitive to acetazolamide, metolazone, or any of their ingredients
- People who have abnormal levels of certain minerals in their blood, particularly low potassium levels
- People with very advanced kidney disease who need dialysis, or whose kidneys are functioning at a very low level
- People who are pregnant or breastfeeding
- People with very low blood pressure (below 90 mmHg on the upper reading), or serious heart conditions present from birth that require surgery
- People who need kidney replacement therapy at any point during their hospital stay
- People with a history of serious imbalances in blood minerals or blood acidity levels
