Who might be able to join this trial:
- People aged 18 years or older
- People whose heart is pumping at a normal or preserved rate (a measure called LVEF of 50% or higher)
- People who have been diagnosed with a type of heart failure called HFpEF — meaning they were admitted to hospital for heart failure in the past 12 months, or have a specific blood marker (NT-proBNP) above a certain level — or people considered at risk of HFpEF because they have more than 2 risk factors such as diabetes, high blood pressure, obesity, or low physical activity
- People who score below 10 on a physical performance test (SPPB), or whose exercise capacity falls below a certain threshold on a breathing and fitness test
- People with a BMI of 28 or above (this applies specifically for entry into the second phase of the trial)
- People who are able to use a mobile phone and a mobile app
Who may not be able to join:
- People who were hospitalised in the month before their first study visit
- People with a history of falling repeatedly
- People with significantly reduced kidney function (eGFR below 20 ml/min/1.73m²)
- People whose heart failure medications were actively changed in the two weeks before their first study visit
- People who are unable to take part in exercise training
- People who are unable to complete a cardiopulmonary exercise test (a supervised test measuring how the heart and lungs respond to physical effort)
- People with severe disease affecting the valves on the left side of the heart
- People with advanced lung disease who require ongoing oxygen therapy
- People who had major surgery within 3 months before screening, or who have major planned surgery scheduled during the study period
- People whose body weight has changed by more than 5% in the 30 days before their first study visit
- People who are pregnant
