Who might be able to join this trial:
- Adults aged 18 or older who have the thinking and communication skills needed to take part in the study
- People who have been diagnosed with fibromyalgia (FM) according to the standard diagnostic criteria set by the American College of Rheumatology (ACR)
- People who have tried non-medication-based treatments for fibromyalgia without enough relief, and who have not yet tried the recommended medications for fibromyalgia
- People whose average pain level over the past 7 days rates 4 or higher on a 0–10 pain scale at the start of the study
Who may not be able to join:
- People who do not wish to take part
- People who are unable to sign the consent form
- People whose fibromyalgia appears to be caused by another condition, such as an underactive thyroid, nutritional deficiencies, diabetes, or a connective tissue disorder
- People who have a psychiatric condition such as schizophrenia, a psychotic disorder, bipolar disorder, or a personality disorder
- People who have obstructive sleep apnea, or who score 3 or higher on a screening tool used to assess sleep apnea risk (called the STOP-Bang questionnaire)
- People who have previously been treated with the medications pregabalin or venlafaxine for any condition
- People who have previously been treated with intravenous ketamine or esketamine for chronic pain
- People who have other painful conditions, such as inflammatory rheumatic diseases
- People who have uncontrolled diabetes, difficult-to-treat high blood pressure, cancer, a specific type of eye pressure condition (narrow-angle glaucoma), an overactive thyroid, serious heart disease, or any other health condition that would make esketamine unsuitable for them
- People with a history of misusing prescription medications, alcohol dependence, or illegal drug use
- People who are pregnant or breastfeeding
- People who are allergic to any of the medications used in the study
