Who might be able to join this trial:

• Adults aged 18 or older who have been diagnosed with coronary heart disease
• People whose heart condition has been assessed as low-risk for cardiovascular events during physical activity, and who also score as low-risk on a standard heart risk assessment tool called the GRACE score
• People who, or whose close family members, are able to use a smartphone and apps
• People who are willing to take part and sign a consent form

Who may not be able to join:

• People who have a blockage of more than 50% in the main artery supplying the heart, or more than 70% blockage in other major heart arteries
• People who are scheduled to have a procedure to restore blood flow to the heart within the next 3 months
• People who have had a heart attack within the past month
• People with severe difficulties in thinking, memory, or understanding
• People whose ability to exercise is severely limited
• People with cancer that has reached an advanced stage
• People whose life expectancy is estimated to be less than 3 months
• People with severe failure of multiple organs in the body
• People whose doctor has determined, after assessment, that home-based exercise rehabilitation would not be appropriate for them for any other reason
• People who are not willing to sign a written consent form
