Who might be able to join this trial:

• People aged between 18 and 70 years
• People who have been diagnosed with hepatocellular carcinoma (a type of liver cancer) confirmed either by tissue testing or according to national clinical guidelines
• People whose liver cancer is classified as an advanced stage (BCLC stage C), which may include cancer that has spread into nearby blood vessels or to other parts of the body, and where the disease is considered suitable for the procedures used in this trial

• People who have at least one tumour that can be measured using standard imaging criteria (RECIST 1.1)
• People whose general physical functioning is at or above a certain level, meaning they are able to carry out basic daily activities (ECOG score of 2 or less)
• People whose expected survival is at least 6 months
• People whose kidney, liver, blood, and other organ functions meet the specific levels required by the trial protocol
• For people being considered for the experimental treatment group: people who have 10 or fewer active tumour sites as seen on a PET-CT scan
• People who have previously received initial systemic (whole-body) treatment and whose liver disease has been stable for at least 3 months
• People who are able and willing to use effective contraception from the time of agreeing to take part until one year after treatment ends
• People who are able to understand the trial information and provide signed consent

Who may not be able to join:

• People who have or have had a second, separate type of cancer (some exceptions may apply — confirm with trial site)
• People whose cancer spread into blood vessels or other parts of the body is considered not suitable for radiotherapy
• People who have already received systemic (whole-body) anti-cancer treatment for their current liver cancer (some prior local treatments may be permitted — confirm with trial site)
• People who have serious problems with the functioning of a major organ that would make treatment unsafe
• People with certain ongoing health conditions that are not well controlled, such as uncontrolled diabetes, an active stomach ulcer, or severe heart or lung disease
• People with an active infection that is not under control, or an active autoimmune disease (where the immune system attacks the body) that requires ongoing systemic treatment
• People with significant problems affecting the nervous system or brain function
• People who are pregnant or breastfeeding, or people who are unable or unwilling to use effective contraception
• People who are known to have an allergy or sensitivity to any of the medicines planned for use in the trial
• People who have any other condition that the trial investigator considers would make participation unsafe or unsuitable
