Who might be able to join this trial:

  • People between 18 and 75 years of age who are able to understand and sign a consent form voluntarily.
  • People who have been diagnosed with early-stage liver cancer (BCLC stage 0 or A), confirmed by tissue testing or approved imaging methods.
  • People who are already scheduled to have surgery to remove the liver cancer with the aim of curing it.
  • People whose results from a special pre-surgery computer analysis (called a PRE score) indicate a higher risk of the cancer coming back aggressively (a score of 0.5 or higher).
  • People whose liver is functioning well, as measured by a standard scoring system called Child-Pugh class A (score of 7 or below).
  • People who are fully active or have only minor limitations in daily activity, as rated on a standard medical scale (ECOG score of 0 or 1).
  • People who have had a standard pre-surgery MRI scan — including multiple imaging phases — taken within one month before joining the trial, with acceptable image quality.
  • People who are willing and able to attend follow-up appointments for at least 2 years.

Who may not be able to join:

  • People whose tissue analysis after surgery shows a type of cancer other than hepatocellular carcinoma (for example, bile duct cancer or a mixed tumour type).
  • People who have had another active cancer within the past 5 years, except for certain low-risk or successfully treated cancers such as some cervical or skin cancers (confirm with trial site).
  • People who pass away or are lost to follow-up within 90 days after surgery.
  • People who have a known allergic reaction to any of the treatment medications used in this trial, which may include oxaliplatin, fluorouracil, PD-1 inhibitors, or lenvatinib.
  • People with serious, uncontrolled health conditions such as severe heart failure, severe kidney problems, or uncontrolled high blood pressure.
  • People who have any other condition that the trial doctor believes would make safe participation or accurate assessment of results difficult.
