Who might be able to join this trial:
- You are 18 years old or older
- You have been diagnosed with Type 1 diabetes for at least 12 months, and your insulin routine has been stable for at least 2 months
- Your HbA1c (a measure of average blood sugar over roughly 3 months) is 8.0% or higher
- You have been diagnosed with gastroparesis (a condition where the stomach empties more slowly than normal), as defined by the National Institutes of Health
- You are willing to use a Medtronic 780G insulin pump system with a Simplera continuous glucose monitor (CGM)
- You are able to give your informed consent before any study activities begin
- If you are placed in the usual care group and already use an insulin pump, you are willing to use it in manual mode only for the whole study
Who may not be able to join:
- You are under 18 years old
- You currently use inhaled insulin (brand name Afrezza)
- You use any blood sugar-lowering medications other than insulin at the time of screening
- You are currently pregnant, breastfeeding, or planning to become pregnant
- You are currently using steroid medications continuously for 2 weeks or more, or expect to need long-term steroid treatment (more than 4 weeks in a row) during the study
- You have a history of a blockage at the stomach outlet or other physical abnormalities in your digestive tract
- Your kidney function is significantly reduced (eGFR below 30) or you are currently on dialysis (confirm with trial site)
- You have had a severe hypoglycemic episode (dangerously low blood sugar requiring help from another person) in the past 3 months
- You have had two or more episodes of diabetic ketoacidosis (DKA) requiring a hospital stay in the past 12 months
- You have a serious medical condition such as severe heart disease, cancer, or chronic liver disease, or a significant mental health condition that the study doctors feel makes participation unsuitable
- You have taken any experimental/investigational drugs within a recent period before screening (confirm timeframe with trial site)
- You currently use cannabis or have a history of cannabinoid hyperemesis syndrome (severe vomiting linked to cannabis use)
