Who might be able to join this trial:
- Adults aged 18 years or older (male or female)
- People who have been admitted to hospital with a diagnosis of acute worsening heart failure, with at least one sign of fluid buildup in the body — such as swelling (edema), fluid in the abdomen (ascites confirmed by ultrasound), or fluid around the lungs (confirmed by chest X-ray or ultrasound)
Who may not be able to join:
- People with Type 1 diabetes
- People with significant kidney disease where the kidneys are functioning at a reduced level (eGFR below 30 mL/min per 1.73 m²), or people who require ongoing dialysis
- People with a systolic (top number) blood pressure reading below 90 mmHg
- People in cardiogenic shock (when the heart is suddenly unable to pump enough blood to meet the body's needs)
- People already taking a medication called acetazolamide on a regular basis
- People who have taken certain medications — specifically an SGLT2 inhibitor, a thiazide, or a thiazide-like water tablet — within the 48 hours before being enrolled in the trial
- People whose heart failure has been triggered by a condition requiring urgent treatment, such as a heart attack, dangerous irregular heart rhythms, or a blood clot in the lungs
- People who are pregnant or breastfeeding
- People with moderate to severe anaemia (low red blood cell levels)
