Carers (people providing unpaid support to someone with heart failure):
- People aged 18 years or older
- People who are physically and mentally able to use a digital device, have conversations with others, and read written materials
- People who regularly provide unpaid care for someone who has been medically diagnosed with heart failure, is receiving standard heart failure treatments, and has had either ongoing heart failure symptoms or at least one episode where symptoms required medical attention in the past 6 months
- People who can speak and understand English
Patients (people living with heart failure):
- People aged 18 years or older
- People who are physically and mentally able to have conversations with others and read written materials
- People who have been medically diagnosed with heart failure, are receiving standard heart failure treatments, and have had either ongoing heart failure symptoms or at least one episode where symptoms required medical attention in the past 6 months
- People who can speak and understand English
- People who receive regular unpaid help managing their condition from a family member, spouse, or close friend
Who may not be able to join:
Carers:
- People who are not able to give informed consent to participate
- People under the age of 18
Patients:
- People who are not able to give informed consent to participate
- People under the age of 18
- People whose carer has declined to take part in this trial
