Who might be able to join this trial:

• People aged between 18 and 75 years old
• People who have been diagnosed with liver cancer (hepatocellular carcinoma), confirmed by specialist imaging (such as a CT scan or MRI) or a tissue biopsy, and whose cancer falls into an early, intermediate (a specific subgroup), or advanced stage according to a standard liver cancer staging system called BCLC 2022 — and who have not received any previous treatment for this cancer
• People who have at least one liver tumour that can be measured using a standard imaging method (called mRECIST)
• People who are generally able to carry out daily activities, ranging from fully active to capable of self-care but unable to do physical work (this is measured on a scale called ECOG, scores 0 to 2)

Who may not be able to join:

• People whose liver cancer has spread widely across both sides of the liver with more than 10 tumour spots and where 50% or more of the liver is affected
• People whose liver cancer has spread into the main blood vessel that carries blood to the liver (the main portal vein), causing a blockage
• People whose liver is not functioning well enough, based on a medical scoring system called Child-Pugh (a score of 10 or above, which is Category C)
• People whose liver function scores poorly on another measure called ALBI Grade 3
• People whose liver cancer is accompanied by blood pressure that is difficult to control, a recent heart attack, or another recent blood clot-related event
• People who are known to have an allergy or bad reaction to a drug called lenvatinib
