Who may be able to join
- People with Parkinson's disease who are being seen at the Center for the Study and Treatment of Parkinson's Disease at the Neuromed Institute of Pozzilli may be considered for recruitment.
- People who show at least 2 of the 4 main signs of Parkinson's disease (tremor, muscle stiffness, slowness of movement, or symptoms that started on one side of the body first), and at least one of those signs must be tremor or slowness of movement.
- People who do not have certain warning signs that might suggest a different condition — such as early balance problems, freezing, memory decline, hallucinations, abnormal involuntary movements, or a specific type of eye movement problem — and whose symptoms are not known to be caused by brain injuries, medications, or toxic substances; people who have also shown a response to standard Parkinson's medications, or who have not yet had an adequate trial of those medications (confirm with trial site).
- People with Alzheimer's disease who are being seen at the Department of Advanced Medical and Surgical Sciences at the University of Campania "L. Vanvitelli" in Naples, or at affiliated Alzheimer's day centres or geriatric facilities, may be considered for screening.
- People with a diagnosis of probable Alzheimer's disease based on the McKhann criteria (2011), confirmed by positive test results for amyloid — either through a brain scan (amyloid PET) or a spinal fluid test.
Who may not be able to join

- People who have a pre-existing psychiatric disorder.
- People who have a neurodegenerative or neurological condition such as multiple sclerosis, amyotrophic lateral sclerosis, Alzheimer's disease, neuromuscular disorders, or epilepsy.
- People who have a diagnosis of dementia.

Primary endpoints
motor symptoms of PD patients will be evaluated with Hoehn and Yahr (HY) score; motor and non motor symptoms of PD patients will be evaluated with MDS-UPDRS; clinical evaluation of cognitive impairment of PD and AD patients; identification of variants/mutations; Identification of metabolomic, lipidomic and proteomic profiles in plasma/serum; Bioinformatic analysis for the identification of biomarkers and molecular pathways associated with PD and /or AD; Identification of epigenetic profiles in patients with PD, AD, and T2D.; Identification of morphological structures through neuroimaging.
