Who might be able to join this trial:

• People who are aged 18 or older
• People who have recently been diagnosed with a type of heart failure called HFrEF (where the heart's pumping function is reduced), whether that diagnosis was made in hospital or at a clinic
• People who have not yet started any standard heart failure medications, or who are taking only one medication that was prescribed for a different health condition
• People who are able to use electronic devices — such as a phone or tablet — to make calls, record measurements like weight and blood pressure, and use digital health tools; if the person themselves finds this difficult, a family member or carer who can help with this is also acceptable
• People whose heart failure is caused by any underlying reason, including blocked arteries, heart valve problems, inherited or infiltrative heart muscle conditions, medication side effects, toxic causes, or a fast heart rate causing heart muscle weakness

Who may not be able to join:

• People who are already taking two or more guideline-based heart failure medications at the time their HFrEF is diagnosed
• People with serious other health conditions or who are medically unstable in a way that requires long-term or ongoing hospital care
• People whose life expectancy is estimated to be less than 12 months
• People who are pregnant or breastfeeding
