Who might be able to join this trial:

• People aged between 18 and 65 years
• People who have attended an orthopaedics and trauma outpatient clinic with neck and/or shoulder pain
• People who have been diagnosed with a specific muscle pain condition called upper trapezius myofascial pain syndrome, confirmed using recognised medical criteria that include a tight band of muscle, tenderness at a specific point, pain that spreads to other areas, and an involuntary muscle twitch response
• People who have exactly one active painful "trigger point" in the upper trapezius muscle (a muscle that runs from the neck to the shoulder)
• People whose symptoms have been present for 4 weeks or longer
• People whose pain rates 4 or higher out of 10 on a standard pain scale
• People who have already tried at least 4 weeks of standard treatments such as physiotherapy and/or pain-relief medication without enough improvement
• People who are able to give written agreement to take part in the trial

Who may not be able to join:

• People who have more than one active trigger point
• People who have been diagnosed with fibromyalgia using the 2010 American College of Rheumatology criteria
• People who have nerve-related neck conditions, such as a pinched nerve or spinal cord compression in the neck area
• People who have had an injection into the same area of muscle within the past 3 months
• People who have a blood clotting disorder, or who are currently taking blood-thinning or anti-clotting medications
• People with a known allergy or medical reason to avoid corticosteroids (anti-inflammatory medicines), local anaesthetics, or components used in platelet-rich plasma (PRP) treatment
• People with an active infection, whether affecting the whole body or a localised area
• People who are pregnant or breastfeeding
• People with a low platelet count (below 100,000 per microlitre of blood), which affects how blood clots (confirm with trial site)
• People who have or have had a cancer diagnosis
• People who are unable to cooperate with the trial process or provide informed consent
