Who may be able to join
- People aged 65 or older who live at home or in an assisted living facility and came to the emergency department after a fall that happened within the last seven days

- The person has an identified caregiver who supports them

Who may not be able to join

- People who live in a nursing home
- People who are admitted to the hospital (rather than being discharged from the emergency department)
- People who have advanced cancer or are receiving hospice care
- People whose injuries make it impossible for them to move around, such as a broken pelvis or a fracture in the leg or foot
