Who might be able to join this trial:

  • You must be 18 years of age or older.
  • Before your heart procedure, a heart scan (echocardiogram) showed that your heart's pumping strength was 40% or below.
  • You have successfully had surgery or a procedure to fix a treatable heart problem, such as heart valve surgery (mitral, aortic, or tricuspid valve), a minimally invasive valve replacement (TAVI), or a procedure to restore blood flow to the heart (bypass surgery or stenting).
  • You joined the study either up to 3 months before your corrective procedure or within 10 days after it.
  • After your procedure, your condition is stable, meaning your blood pressure is adequate, your heart rate is in a normal range (50–110 beats per minute), you are not on drips for your heart or fluid removal, and you show no signs of fluid build-up.
  • If you were already taking certain heart medications called ARNI or SGLT2 inhibitors, you must stop taking them for at least one week before joining the trial.
  • You must be willing and able to sign a consent form agreeing to take part.

Who may not be able to join:

  • You have a history of serious abnormal heart rhythms (sustained ventricular tachycardia or ventricular fibrillation).
  • After your valve surgery, there is more than a moderate amount of blood leaking around or through the valve.
  • If you had a procedure to restore blood flow to your heart, there are still significant blockages remaining in your heart arteries.
  • After bypass surgery, a scan has shown that one of the new grafts is blocked.
  • You are planning to become pregnant during the study period.
  • Your blood pressure remains too high (above 160 mmHg) even with medication.
  • Your kidneys are not working well enough, based on a blood test that measures kidney filtering ability (confirm with trial site).
  • You are unable to tolerate a specific combination heart medication called sacubitril/valsartan, for example due to low blood pressure, a history of severe allergic swelling (angioedema), or a certain type of kidney artery narrowing.
  • You are unable to tolerate a type of diabetes medication called SGLT2 inhibitors — for example, if you have type 1 diabetes or have had repeated episodes of a serious condition called diabetic ketoacidosis.
  • You have any other health condition that the study doctor feels would make it difficult for you to safely participate or be followed up during the trial.
