  • People whose most recent A1C result was not recorded or is missing.
  • People whose most recent A1C test was done within the last 12 months from the date the data was pulled.
  • People who fall into the "Score-Based Glycemic Risk Group" category may not be eligible — this includes the following:
  • People with a risk score below 9.
  • People whose most recent A1C test was done within the last 30 months from the date the data was pulled.
  • People whose most recent A1C result was above 5.7, indicating prediabetes or diabetes.
