Who may be able to join

  • People between 25 and 75 years old.
  • People who have had type 2 diabetes for more than 1 year.
  • People who speak English or Spanish.
  • People whose A1c (a blood sugar level measured over the past few months) was 8% or higher within the past 6 months — the trial team may adjust this requirement depending on how recruitment is going.
  • People whose kidneys are functioning above a certain level (a blood test called eGFR must be above 30 — confirm with trial site if unsure what this means for you).
  • People who are willing to wear a small sensor that tracks blood sugar continuously, work with a community health worker (including home visits), and work with a clinical pharmacist.

Who may not be able to join

Each point below is a reason the trial team may not be able to accept someone. It is not a list of requirements to meet.

  • People who are planning to have weight loss surgery or take prescription medication specifically for weight loss (not including GLP-1 medications).
  • People who are currently using, have recently used, or are planning to use a continuous glucose monitor (CGM — a wearable blood sugar tracking device).
  • People who are currently taking steroid medications.
  • People who are currently misusing illegal drugs.
  • People who are pregnant or planning to become pregnant.
  • People who have certain other serious health conditions, such as a recent heart attack, recent stroke or mini-stroke, certain cancers, certain blood disorders, or severe low blood sugar episodes (confirm with trial site for the full list).
  • People who have a psychiatric condition that may make it difficult to complete the tasks required by the study.
  • People who have skin changes that would prevent wearing the glucose sensor, or who are allergic to adhesives.
  • People who are planning to move out of the local area within 12 months, or who live more than 20 miles from the trial site.
  • People who are currently taking part in another research study.
