Who may be able to join
- People aged 19 years or older
- People who have been diagnosed with type 2 diabetes
- People who identify as Black (self-reported)
- People who receive their care at one of the study sites and have had at least one visit there in the past year
- People who have a moderate to severe level of diabetes-related emotional distress, based on a score of 2 or higher on a standard diabetes distress questionnaire
