  • The person has been diagnosed with heart failure.
  • The person has been diagnosed with dementia.
  • The person has been diagnosed with schizophrenia.
  • The person has shown an inability to receive and respond to a text message.
  • The person does not manage their own medications — for example, if someone else administers their medication for them.

For the supporting friend or family member:

  • The support person has shown an inability to receive and respond to a text message.
  • The support person is unable to communicate by phone.
