Who may be able to join
- People who are 18 years of age or older
- People whose systolic (top number) blood pressure reading is 130 mmHg or higher, confirmed across at least two separate visits at least one day apart but within seven days, based on an average of six measurements total

- People who have a serious long-term or terminal illness where managing blood pressure and/or blood sugar through regular primary care would not be clinically appropriate, including those with end-stage kidney disease
- People who are confined to bed
- People who are planning to move away from the study area within the next two years
- People who are unwilling to follow the requirements of the study protocol
