Who may be able to join
- Adults between 18 and 70 years of age.
- People who have been diagnosed with type 2 diabetes.
- People whose HbA1c blood sugar level (tested within the last 6 months) is 7% or higher — this applies whether or not diabetes medications (pills or injections) are currently being taken.
- People who score below the published average on a questionnaire measuring how they cope with diabetes (the Acceptance and Action Diabetes Questionnaire), or who score below the published average on at least 2 areas of a diabetes self-management questionnaire (the Summary of Diabetes Self-Care Activities).
- People who identify as Hispanic, Latino, Latina, or Latinx.
- People whose preferred language is English or Spanish.
- People who receive their regular care at the clinic where the study is being conducted.
Who may not be able to join
Each point below is a reason the trial team may not be able to accept someone. It is not a list of requirements to meet.
- People with end-stage kidney disease who are currently on dialysis.
- People who have a medical condition or life situation that would make taking part in the trial unsuitable (confirm with trial site).
- People with an advanced form of diabetes-related eye disease (proliferative diabetic retinopathy) that would prevent participation.
- People who are unable to read or understand the consent forms or study instructions.
- People who are pregnant or who plan to become pregnant within the next 6 months.
